Pigtail straightener left in ascending aorta

Ravaux Justine Mafalda*; Amond Laurent; Remy Philippe; Ghammad Kaoutar; Dasnoy Denis; Swaelens Charles

A case of retained Intra-Vascular Foreign Body (IVFB) after successful Thoracic Endovascular Aortic Treatment (TEVAR) for an isthmic penetrating atherosclerotic ulcer is reported. Thoracic pain on post-operative day one required angioscanner (CTA) investigations and showed a longilineal image from aortic valve to brachio-cephalic origin, confirmed by Trans-oesophageal Echo-Endoscopy (TEE).

Infective endocarditis in pediatric patients in a tertiary care hospital, Pakistan

Arif Maqsood Ali*; Azhar Mehmood Kayani; Shazia Arif; Agha Babar Hussain

Introduction: Endocarditis is a rare disorder which is associated with significant morbidity & mortality world over especially in the developing countries. The etiology of disease has gradually shifted from Rheumatic heart disease to prosthetic & device associated endocarditis in the developed world. Very little is known about its etiology in Pakistan.

Risk stratification in early stage arrhythmogenic cardiomyopathy and provocable brugada ECG

Stefan Peters

Risk factors of malignant tachyarrythmias should be assessed in cases with early stage arrhythmogenic cardiomyopathy and provocable Brugada ECG. Spontaneous right precordial coved-type ST segment elevation, tall R wave in lead a VR, QRS fragmentation, 1° AV block, inducibility during electrophysiological examination, and localised right precordial QRS prolongation should be tested in 20 out of 128 patients.

The klotho gene and soluble klotho in health and disease: From 1997-2018; A review

Moran S Saghiv

The topic of aging and anti-aging has interested humanity from the start of time [1]. In Greek myth (According to Hesiod's Theogony), the goddess Clotho spins the thread of life; the goddess Lakhesis determines the length of the thread; and the goddess Atropos cuts the thread [2]. It is obvious that ageing is a multifactorial process that influences many aspects of our existence as humans and is influenced by many factors [3]. Such factors include biological, environmental, psychological, lifestyle, and medical.

The effects of partial sleep deprivation and the sub-maximal NDKS exercise testing protocol on S-Klotho and hemodynamic responses in men

Saghiv M; Backes B; Cook K; Frank S; Schaaf J; Wagner M

It has long been known that different extents of sleep deprivation (partial and/or full) have negative implications on function, hemodynamic, and physiological responses [1-6]. The influences of sleep deprivation may in some cases, be sex-specific (male vs female). Such influences may be characterized by neurologic, endocrine, physiologic-metabolic, and behavioral changes, often for worse [7-9].

Analysis of post-operative systemic to pulmonary artery shunt failure

Thorpe MH; Holinski PM; Al Aklabi M; Bauman ME; Ryerson LM*

Background: Systemic to pulmonary artery shunts are utilized in the palliation of children with congenital heart disease and limited or ductal dependent pulmonary blood flow.

Tramadol linked QT interval prolongation and Torsades de pointes

Jules C Hancox

Tramadol is a synthetic opiate used in the treatment of pain. This article provides a brief overview of the evidence for QT interval prolongation and Torsades De Pointes (TdP) arrhythmia with tramadol. Data from a study of patients with isolated tramadol toxicity showed rate corrected QT (QTc) interval prolongation in ~25% of patients, whilst in a distinct study of drug-induced QTc prolongation, QTc prolongation was observed with tramadol, with the extent of QTc change correlating well with plasma concentrations of the drug. Evaluation of publically available information on the EudraVigilance database of suspected adverse events found 3 cases of long QT syndrome and 7 cases of TdP with the drug.

Bicuspid aortic valve disease: A simple embryonic defect or a complex syndrome? Paradigm or certainty?

Carmela Rita Balistreri

Cardiovascular diseases (CVDs) show a growing incidence in Western populations due to ongoing ageing [1,2]. Precisely, it is estimated that the CVD incidence will reach 36.9% to 40.5% by 2030, without changes in prevention or treatments [3]. Thus, CVDs are a very challenge. However, advances have been made in recent years in the management of some CVDs, such as ischemic heart diseases [3]. Differently, for a large CVD group, the management and the outcome still are complex. Typical examples are the aneurysms with heterogeneous etiology and increasing incidence, especially in elderly people [4].

Heart failure with preserved ejection fraction(HF-PEF): A mini-review

Peysh A Patel*; Noman Ali

Heart Failure with Preserved Ejection Fraction (HF-PEF) describes patients with classic symptoms but without left ventricular systolic impairment (ejection fraction ≥ 50%). These cohorts have features of diastolic dysfunction which is considered a precursor and associated with poor outcomes. HF-PEF appears to be of increasing prevalence, which may relate to an ageing population as well as improved recognition of the disorder. It is prudent to explore underlying aetiology, which is usually a primary myocardial disorder.

Stem cell therapy: An emerging treatment approach for atrial fibrillation

Liang Hong

Atrial fibrillation (AF) is the most common arrhythmia worldwide; it is associated with increased risk for stroke, heart failure, inconsistent blood supply, and additional heart rhythm problems. Despite recent advances in antiarrhythmic drugs (AADs) therapies, variability in response to AADs in individual patients is caused either by heterogeneity of the underlying electrical substrate in patients or by our inability to select mechanism-based therapies.
