Dichorionic Diamniotic Twin Pregnancy with Single Fetal Demise

Clinical image description

A 25 year old lady with 32 weeks of twin pregnancy came to the Gynecology and Obstetric department at Owaisi Hospital, Hyderabad for regular antenatal checkup in August 2019. She was asked to take up Ultrasound Growth scan. On ultrasound it showed discordant growth of the twins. Antenatal doppler was suggestive of twin B demise (no fetal pulsations were seen) whereas twin A was normal. She was admitted in the hospital and monitored regularly to prevent complications to her as well as to the surviving twin. Investigations like antenatal doppler, coagulation test and complete blood picture were normal.

At 37 weeks of gestational age she underwent lower segment caesarean section. Twin A was alive normal healthy male baby with 2.8 kg of weight and Apgar score of 8 whereas twin B was dead and macerated.

She was followed up along with her live baby weekly twice for about a month. Investigations like complete blood picture and coagulation test were repeated with normal outcomes. Breast feeding and baby’s growth was also normal thus both mother and child were healthy.

This was a rare case of Dichorionic Diamniotc twin pregnancy with single fetal demise which was confirmed at 32 weeks of gestational age thus it was carefully managed by monitoring mother and the surviving child.
