Neuropathic pain in chronic idiopathic axonal neuropathy: Fast pain reduction after topical phenytoin cream

    Pain in Chronic Idiopathic Axonal Neuropathy (CIAP) is often treated based on guidelines, not specifically developed for CIAP patients, and we can only extrapolate findings from for instance Painful Diabetic Neuropathy (PDN) to our CIAP patients. Clearly, the pathogenesis and pathophysiology of CIAP will be different from that of most PDN patients. Many CIAP patients complain of the intolerable side-effects of current analgesic medication. In order to help those patients, we developed a treatment-regime consisting of the supplement palmitoylethanolamide and topical formulations of analgesics.


Recombinant human bone morphogenetic protein-2 for correction of unilateral cleft lip and palatal: Case report

    The aim of this paper was to present a case report in which a unilateral cleft lip and palate was repaired with bone graft. The 11 years old Caucasian female patient was subjected to correction of unilateral cleft lip and palatal left (left side) with the rhBMP-2 (INFUSE® Bone Graft). Bone reconstruction and including canine repositioning was planned. A one and a half mm titanium mesh (was fixed by 4 titanium screws in the adjacent bone structures), to sustain collagen sponge with the protein stays in the fissure, allowing a framework and a conformation of the alveolar process.


Pain in chemotherapy-induced peripheral neuropathy: NGF and the logic of topical phenytoin cream

    In Chemotherapy-Induced Peripheral Neuropathy (CIPN) epidermal nerves and keratinocytes are damaged due to toxic influences of oncolytical therapies. One important detrimental factor recently described is the Nerve Growth Factor (NGF). NGF is produced by irritated keratinocytes, and NGF over production leads to over active nociceptors. This mechanism of peripheral sensitization induces localized peripheral pain. Both membranes of keratinocytes and nociceptors are rich in sodium channels.


Oral apthous ulcer healing steps

    27 years old male patient presenting to the primary health care clinic with chief complaint of painful ulcer on the lower lip of 3 days duration. There was Apthous ulcer measuring 07cm on the patient’s lower lip, which was provisionally diagnosed as an oral apthous ulcer. These photographs were taken on daily basis as a follow up and it excel in demonstrating the healing process of the apthous ulcer without the use of any topical or systemic drug.


Supposed otolaryngology emergency which presents late: Bilateral congenital choanal atresia

    Bilateral congenital choanal atresia is a life threatening emergency which always presents in new born. Cyclical respiratory distress, cyanosis and apnoea relieved by crying are common clinical features. We report a rare case of bilateral choanal atresia in a 5 year old girl with unremarkable neonatal history, whom presented to us with persistent bilateral nasal obstruction.

Atypical scrotal trauma in a child

    Scrotal trauma is end result of blunt as well as penetrating trauma. Although very rarely reported in literature, penetrating trauma is often secondary to civilian or military violence. Except in cases of scrotal avulsion, this trauma usually manifests as scrotal swelling with intratesticular and scrotal hematoma and various degrees of scrotal wall ecchymosis. Clinical presentation is immediate in majority of cases for penetrating wounds.


An unusual finding of dilated stomach in the pelvic cavity secondary to gastric outlet obstruction by cholangiocarcinoma

    Cholangiocarcinomas is a rare malignancy, often presenting in advanced, difficult-to-treat stages because of varied locations in the biliary tree.
This case describes a 78 year-old female who presented with history of anorexia, significant loss and intractable vomiting. On imaging there was extensive gastric dilatation secondary to gastric outlet obstruction to a degree that the stomach reached the pelvic cavity.
